2 – Build your Free Trial Survey

Download the Application
Go to the Downloads folder and look for this icon
Move the Icon to your Home Folder
Select Create a new survey or use a survey template
If you prefer, you can also fax your trial survey/form to 954-252-4127 or e-mail to survey@berryforms.com and we will draft a demo survey for you ( conditions apply )
