How to Purchase your eSurvey License :
1 – Enter your billing information and process your Payment
If you purchased the license through Blackberry's Apps World - go to Step 3
- PLEASE PROCEED TO SELECT WHICH AND HOW MANY LICENSES YOU WANT TO PURCHASE USING PAYPAL'S SHOPPING CART ( YOU CAN ALSO USE YOUR CREDIT CARD IF YOU DO NOT HAVE A PAY PAL ACCOUNT )
If you do not feel comfortable with online payment, please CALL (877) 436-8133 or send us an E-MAIL TO BILLING@CLOUDMOBILEFORMS.COM with your particular needs.
Select from the drop down menu your main license and then proceed to add all your necessary licenses with quantities at PayPal's shopping cart.
2 – How to proceed after having paid with PayPal or Credit Card
- Once your order is completed, you will receive a confirmation payment e-mail from PayPal and an Upgrade Instructions e-mail from our side
- Please be aware that if your purchase after hours or during weekends or holidays, then your confirmation e-mail with instructions will be delivered the next working day. Hour business hours are Monday through Friday from 8:00 am to 6:00 pm EST
- If you prefer, you can also fax your order and payment details to our secure fax 954-252-4127 or e-mail a scanned copy to billing@cloudmobileforms.com
3 –Activate your Licenses
- Each Blackberry will have to be activated individually.
- To activate the license we will require the following information
- PIN & Mobile Number
- Blackberry Model
- E-Mail address from the subscriber
- If you are activating more than 2 licenses all together, then please proceed to fill out the Activation Request Form

- You can also activate your license directly from your Blackberry as follows :
- Open eSurvey
- Click the Menu Button
- Scroll down and select the option " Activate License "
- An email message will open . Please put your PayPal Transaction ID and hit send.
- You will be receiving an email with your License Key and further instructions.
4 – Additional Services and Benefits
- With each Enterprise license you will be entitled to a Box.net Lite FREE account
- With each Enterprise license you will be entitled to a Zoho Reports/Creator FREE account
- Professional Services are quoted on a consulting basis and billed separately
- Business Intelligence module will be billed as a separate service
